Headset and Speaker handling while in a Call

Description

At least in SHR, Speaker doesn't work correctly because of bad state file, and headset doesn't work because it's never even loaded. I submitted a bug report in SHR and also a fix, thought it might be useful here as well: ​http://shr.bearstech.com/trac/ticket/377

Please fix it up stream.
